Create Your Account: This option will appear first when you select the "continue" button above. You will need to set your email address and password.

Step 2:

After your account is created, you will be taken to your profile page where you will need to enter in your information within 4 sections. * Be sure to select the "update" button at each stage to save.

Step 3:

Once all of your information is updated, you will see the green text at the top confirming that you are ready. 

Step 4: 

Simply select the button that says, "Start this Process." - You will get a file emailed to you.
